Oxalate (mg): the primary dietary driver of calcium oxalate stone formation
About 80% of kidney stones are calcium oxalate. Dietary oxalate is absorbed and excreted in urine; high concentrations promote crystal formation. Personal daily limits (typically 50 to 100 mg) are set by a clinician based on 24-hour urine results.
Clinical note
Calcium consumed with meals binds oxalate in the gut before it is absorbed, which actually reduces stone risk. Never restrict calcium without guidance.

Purines (mg): metabolise to uric acid and crystallise in joints
Purines break down into uric acid. When uric acid exceeds its solubility threshold, crystals deposit in joints, causing sudden, severe attacks. Animal-source purines raise uric acid significantly more than equivalent plant sources.
Clinical note
Fructose (in sugary drinks and juice) also raises uric acid independently of purines. Adequate hydration and low-fat dairy intake have a protective effect.

Potassium and phosphorus: both accumulate when kidneys are moderately impaired
At Stage 3, kidneys may struggle to clear excess potassium and phosphorus. High potassium affects heart rhythm; excess phosphorus depletes bone mineral density. Limits are set by a renal dietitian based on blood results.
Clinical note

Gluten (allergen): even trace amounts trigger intestinal immune damage
An autoimmune condition triggered by gluten (a protein in wheat, barley, and rye). Immune activation damages intestinal villi and impairs nutrient absorption. There is no safe threshold; lifelong strict avoidance is required.
Clinical note
Certified gluten-free oats are safer but still cause reactions in a small subset of people with coeliac disease. Introduce carefully.

Lactose (allergen): milk sugar that insufficient lactase cannot break down
Insufficient intestinal lactase means lactose reaches the colon undigested, where gut bacteria ferment it, causing gas, bloating, and diarrhoea. Tolerance thresholds vary widely: some people manage small amounts, others cannot tolerate any.

FODMAPs are poorly absorbed short-chain carbohydrates rapidly fermented by gut bacteria, causing bloating, pain, and altered bowel habit in IBS. The low-FODMAP diet is followed in structured elimination and reintroduction phases under a dietitian.
